1.    INTRODUCTION AND SCOPE  <\/span>\u4ecb\u7ecd\u4e0e\u8303\u56f4<\/span><\/strong><\/span><\/p>\n

1.1.  Background <\/span>\u80cc\u666f<\/span><\/strong><\/span><\/p>\n

Growing antimicrobial resistance (AMR) linked to the discharge of drugs and particular chemicals into the environment is one of the most worrying health threats today, according to research by UN Environment (1).  AMR accounts for an estimated 700 000 deaths per year and, by 2030, will represent up to US$ 3.4 trillion in Gross Domestic Product (GDP) loss (2). AMR has been identified as a priority at the World Health Assembly since 1998 (3), with rising momentum throughout the years. Since 1998, there have been a series of resolutions on AMR. This paved the wayto the Sixty-eighth World Health Assembly in May 2015, where the World Health Assembly endorsed a global action plan to tackle AMR, including antibiotic resistance, the most urgent drug resistance trend (4).  More recently, the Thirteenth General Programme of Work (2019-2023) highlighted the need to address this emerging threat under the section for \u00abTackling antimicrobial resistance\u00bb (2).  It is only recently that the need to address waste and waste water management from pharmaceutical production has been explicitly addressed. Namely, on 30 November 2018, the World Health Organization\u2019s (WHO) Executive Board meeting decided that technical input will be provided to Good Manufacturing Practice (GMP) guidance on waste and wastewater management from the production of Critically Important Antimicrobials (5, 6). This \u201cpoints to consider\u201d document was written furtherto this recent decision.<\/span><\/p>\n

\u6839\u636e<\/span>UN<\/span>\u73af\u5883\u7814\u7a76\uff0c\u5f53\u524d\u7531\u4e8e\u836f\u54c1\u5c24\u5176\u662f\u5316\u5b66\u54c1\u6392\u653e\u81f3\u73af\u5883\u4e2d\u800c\u65e5\u76ca\u589e\u957f\u7684\u6297\u751f\u7d20\u8010\u836f\u6027\uff08<\/span>AMR<\/span>\uff09\u5df2\u6210\u4e3a\u6700\u4ee4\u4eba\u62c5\u5fe7\u7684\u5065\u5eb7\u5a01\u80c1\u3002<\/span>AMR<\/span>\u6bcf\u5e74\u9020\u6210\u7ea6<\/span>70<\/span>\u4e07\u4eba\u6b7b\u4ea1\uff0c\u5230<\/span>2030<\/span>\u5e74\u5c06\u9020\u6210\u9ad8\u8fbe<\/span>3.4<\/span>\u4e07\u4ebf\u7f8e\u5143<\/span>GDP<\/span>\u635f\u5931\u3002\u81ea<\/span>1998<\/span>\u5e74\u4ee5\u6765\uff0c<\/span>AMR<\/span>\u5df2\u88ab<\/span>WHA<\/span>\u4f5c\u4e3a\u4f18\u5148\u4e8b\u52a1\u6301\u7eed\u63a8\u8fdb\u3002\u81ea<\/span>1998<\/span>\u5e74\u4ee5\u6765\uff0c\u5728<\/span>AMR<\/span>\u65b9\u9762\u5df2\u6709\u4e86\u4e00\u7cfb\u5217\u7684\u89e3\u51b3\u65b9\u6848\u3002\u8fd9\u4e3a<\/span>2015<\/span>\u5e74<\/span>5<\/span>\u6708\u7b2c<\/span>68<\/span>\u6b21<\/span>WHA<\/span>\u94fa\u5e73\u4e86\u9053\u8def\uff0c\u4f1a\u8bae\u4e2d<\/span>WHA<\/span>\u901a\u8fc7\u4e00\u9879\u5168\u7403\u8ba1\u5212\u5e94\u5bf9<\/span>AMR<\/span>\uff0c\u5305\u62ec\u6297\u751f\u7d20\u8010\u836f\u6027\u3001\u6700\u7d27\u6025\u836f\u7269\u8010\u836f\u6027\u8d8b\u3002\u6700\u8fd1\u7684\u7b2c<\/span>13<\/span>\u6b21\u5e38\u52a1\u5de5\u4f5c\u8ba1\u5212\uff08<\/span>2019-2023<\/span>\uff09\u5728<\/span>\u201c<\/span>\u5e94\u5bf9\u6297\u751f\u7d20\u8010\u836f\u6027<\/span>\u201d<\/span>\u7ae0\u8282\u5f3a\u8c03\u4e86\u89e3\u51b3\u6b64\u7d27\u6025\u5a01\u80c1\u7684\u9700\u8981\u3002\u76f4\u5230\u6700\u8fd1\u624d\u6e05\u695a\u63d0\u51fa\u4e86\u836f\u54c1\u751f\u4ea7\u4e2d\u5e9f\u6c34\u5e9f\u7269\u7ba1\u7406\u7684\u9700\u8981\u3002\u4e5f\u5c31\u662f\u8bf4\uff0c\u5728<\/span>2018<\/span>\u5e74<\/span>11<\/span>\u6708<\/span>30<\/span>\u65e5\uff0c<\/span>WHO<\/span>\u6267\u884c\u8463\u4e8b\u4f1a\u8bae\u51b3\u5b9a\u9488\u5bf9\u7279\u522b\u91cd\u8981\u6297\u83cc\u5242\u7684\u751f\u4ea7\u63d0\u51fa\u5e9f\u7269\u5e9f\u6c34\u7ba1\u7406\u65b9\u9762\u7684<\/span>GMP<\/span>\u6307\u5357\u3002\u672c\u8003\u91cf\u8981\u70b9\u6587\u4ef6\u662f\u5bf9\u6700\u8fd1\u51b3\u5b9a\u7684\u54cd\u5e94\u3002<\/span><\/span><\/p>\n

This document is to be considered as a time-limited document that addresses the current needs for guidance on how GMPs should be implemented to waste and waste water management for production of antimicrobials, with a focus on Critically Important Antimicrobials. Wherever possible, this text is informedby relevant evidence. However, the evidence base may be weak in some areas, therefore inputs from stakeholders and experts could be beneficial.<\/span><\/p>\n

\u672c\u6587\u5e94\u4f5c\u4e3a\u6682\u65f6\u89e3\u51b3\u5f53\u524d\u6307\u5357\u9700\u6c42\u7684\u6587\u4ef6\uff0c\u6307\u5bfc\u5982\u4f55\u5b9e\u65bd\u6297\u83cc\u5242\u751f\u4ea7\u4e2d\u7684\u5e9f\u7269\u548c\u5e9f\u6c34\u7ba1\u7406<\/span>GMP<\/span>\uff0c\u91cd\u70b9\u5728\u4e8e\u7279\u522b\u91cd\u8981\u7684\u6297\u83cc\u5242\u3002\u867d\u7136\u672c\u6587\u5c3d\u53ef\u80fd\u91c7\u7528\u76f8\u5173\u8bc1\u636e\uff0c\u4f46\u8fd9\u4e9b\u8bc1\u636e\u53ef\u80fd\u5728\u67d0\u4e9b\u9886\u57df\u6bd4\u8f83\u8584\u5f31\uff0c\u56e0\u6b64\u5e72\u7cfb\u4eba\u548c\u4e13\u5bb6\u4eec\u5982\u80fd\u63d0\u51fa\u5efa\u8bae\u5c06\u5927\u6240\u88e8\u76ca\u3002<\/span><\/span><\/p>\n

1.2.  Purpose<\/span>\u76ee\u7684<\/span><\/strong><\/span><\/p>\n

Thepurpose of this document is to: <\/span>\u672c\u6587\u4ef6\u7684\u76ee\u7684\u662f<\/span><\/span><\/p>\n

\u2022      Raise awareness of medicines\u2019 manufacturers, GMP inspectors and inspectorates in all Member States on sections of relevant GMP guidance that are applicable to the management ofwaste\/waste water from the production of antimicrobials.<\/span><\/p>\n

\u2022      <\/span>\u63d0\u9ad8\u836f\u54c1\u751f\u4ea7\u5546\u3001<\/span>GMP<\/span>\u68c0\u67e5\u5458\u548c\u6240\u6709\u6210\u5458\u56fd\u68c0\u67e5\u56e2\u961f\u5728\u9002\u7528\u4e8e\u6297\u83cc\u5242\u751f\u4ea7\u7684\u5e9f\u7269<\/span>\/<\/span>\u5e9f\u6c34\u7ba1\u7406\u76f8\u5173<\/span>GMP<\/span>\u6307\u5357\u65b9\u9762\u7684\u610f\u8bc6\u3002<\/span><\/span><\/p>\n

\u2022      Provide clarification on the interpretation of those clauses and specific measures that should be taken tobe considered compliant with the relevant sections of GMP guidance.<\/span><\/p>\n

\u2022      <\/span>\u5bf9\u8fd9\u4e9b\u6761\u6b3e\u89e3\u91ca\u63d0\u4f9b\u6f84\u6e05\uff0c\u4ee5\u53ca\u5728\u7b26\u5408<\/span>GMP<\/span>\u6307\u5357\u76f8\u5173\u90e8\u5206\u65f6\u5e94\u8003\u8651\u91c7\u53d6\u7684\u5177\u4f53\u63aa\u65bd\u63d0\u4f9b\u89e3\u91ca\u3002<\/span><\/span><\/p>\n

\u2022      Raise awareness of medicine\u2019s manufacturers, GMP inspectors and inspectorates, on the importance of considering all aspects of GMP implementation and to also focus on the parts of GMP that may not have a direct product quality impact.<\/span><\/p>\n

\u2022      <\/span>\u63d0\u9ad8\u836f\u54c1\u751f\u4ea7\u5546\u3001<\/span>GMP<\/span>\u68c0\u67e5\u5458\u548c\u68c0\u67e5\u56e2\u961f\u8003\u8651<\/span>GMP<\/span>\u5b9e\u65bd\u6240\u6709\u65b9\u9762\u91cd\u8981\u6027\u7684\u610f\u8bc6\uff0c\u540c\u65f6\u5173\u6ce8\u53ef\u80fd\u5bf9\u836f\u54c1\u8d28\u91cf\u4ea7\u751f\u76f4\u63a5\u5f71\u54cd\u7684<\/span>GMP<\/span>\u90e8\u5206\u3002<\/span><\/span><\/p>\n

\u2022      Raise awareness of Member States, to establish and enforce requirements for their local pharmaceutical production facilities to safely dispose of the waste and waste water that is generated while manufacturing antimicrobials, with a focus on Critically Important Antimicrobials.<\/span><\/p>\n

\u2022      <\/span>\u63d0\u9ad8\u6210\u5458\u56fd\u5efa\u7acb\u548c\u6267\u884c\u5176\u672c\u5730\u836f\u54c1\u751f\u4ea7\u8bbe\u65bd\u7684\u610f\u8bc6\uff0c\u4ee5\u5b89\u5168\u5904\u7406\u6297\u83cc\u5242\u751f\u4ea7\u8fc7\u7a0b\u4e2d\u4ea7\u751f\u7684\u5e9f\u7269\u548c\u5e9f\u6c34\uff0c\u540c\u65f6\u5173\u6ce8\u7279\u522b\u91cd\u8981\u7684\u6297\u83cc\u5242\u3002<\/span><\/span><\/p>\n

\u2022      Provide proposals on what should be done by the different stakeholders in order to help control and reduce contamination of the environment with antimicrobials and related chemicals coming from pharmaceutical production processes.<\/span><\/p>\n

\u2022      <\/span>\u4e3a\u4e0d\u540c\u5e72\u7cfb\u4eba\u63d0\u4f9b\u884c\u52a8\u65b9\u6848\uff0c\u4ee5\u5e2e\u52a9\u63a7\u5236\u548c\u964d\u4f4e\u73af\u5883\u4e2d\u6765\u81ea\u836f\u54c1\u751f\u4ea7\u5de5\u827a\u7684\u6297\u83cc\u5242\u548c\u76f8\u5173\u5316\u5b66\u54c1\u6c61\u67d3\u3002<\/span><\/span><\/p>\n

\u2022      Discuss options and tools to reduce and mitigate the uncontrolled disposal of waste and waste water containing antimicrobials when manufacturing medical products, with a focus on how GMP can more comprehensively address environmental aspects in the prevention of AMR, including the potential role of inspectors to tackle this issue. This includes:<\/span><\/p>\n

\u2022      <\/span>\u8ba8\u8bba\u964d\u4f4e\u548c\u7f13\u89e3\u836f\u54c1\u751f\u4ea7\u4e2d\u542b\u6297\u83cc\u5242\u7684\u5e9f\u7269\u4e0e\u5e9f\u6c34\u4e0d\u53d7\u63a7\u5904\u7406\u7684\u65b9\u6cd5\u548c\u5de5\u5177\uff0c\u540c\u65f6\u5173\u6ce8<\/span>GMP<\/span>\u5982\u4f55\u80fd\u66f4\u5168\u9762\u89e3\u51b3\u73af\u5883\u4e2d<\/span>AMR<\/span>\u9884\u9632\u95ee\u9898\uff0c\u5305\u62ec\u68c0\u67e5\u5458\u5e94\u5bf9\u6b64\u95ee\u9898\u7684\u53ef\u80fd\u8eab\u4efd\u3002\u5176\u4e2d\u5305\u62ec\uff1a<\/span><\/span><\/p>\n

o     a presentation of a pilot process in the WHO Prequalification (PQT) Inspectorate to comprise verification of adequate preventive measures in place to  prevent environmental contamination with Critically Important Antimicrobials manufactured at medicines manufacturing facilities, involving both active pharmaceutical ingredient (API) and finished pharmaceutical product (FPP) production facilities;<\/span><\/p>\n

o     <\/span>\u5728<\/span>WHO<\/span>\u9884\u8ba4\u8bc1\uff08<\/span>PQT<\/span>\uff09\u68c0\u67e5\u5f00\u59cb\u9636\u6bb5\u4e2d\u5305\u62ec\u5bf9\u9884\u9632\u63aa\u65bd\u662f\u5426\u5145\u5206\u7684\u6838\u67e5\u4ee5\u9884\u9632\u5728\u836f\u54c1\u751f\u4ea7\u573a\u6240\u4e2d\u6240\u751f\u4ea7\u7684\u7279\u522b\u91cd\u8981\u7684\u6297\u83cc\u5242\u73af\u5883\u6c61\u67d3\uff0c\u5305\u62ec<\/span>API<\/span>\u4e0e<\/span>FPP<\/span>\u751f\u4ea7\u573a\u6240\u3002<\/span><\/span><\/p>\n

o     a discussion of a proposal to update GMP guidelines, with a focus on the guidelines WHO good manufacturing practices forpharmaceutical products containing hazardous substances (Annex 3, TRS957, 2010)(7);<\/span><\/p>\n
